Standing Desks: What the Trials Actually Show (And How to Use One)

Sit-stand desks are the rare workplace intervention tested in genuine randomized trials — and the results are usefully modest: reduced daily sitting time (~30–120 minutes), modest improvements in comfort and some cardiometabolic markers, no weight-loss miracles, and a clear pattern in who benefits: the people who actually alternate. The desk is a tool; the schedule is the intervention.

What the evidence says

OutcomeTrial evidence
Sitting timeReduced 30–120 min/day consistently
Back/neck comfortSmall-to-moderate improvement, strongest in those with pre-existing discomfort
Glucose/lipidsModest; breaking sitting matters more than standing per se
Calories / weightNegligible — ~0.15 kcal/min difference
ProductivityNo degradation for typing-heavy work once accustomed (typing slows briefly at first)

The ramp-up that prevents the hate

Most standing-desk abandonment is a ramping injury: week one at four hours standing buys calf aches, plantar pressure, and a desk permanently lowered by week two. The tolerable ramp: 15–30 minutes per standing block, 2–4 blocks daily, adding one block every 3–4 days — full alternation inside two weeks. Anti-fatigue mat, cushioned shoes, and a foot rail (alternate feet) carry the long blocks.

Presets: the two numbers that make it stick

A sit-stand desk whose heights must be re-discovered daily does not get moved. Compute both presets once — seated elbow height and standing elbow height (the desk height calculator does it) — program them into the desk's memory, and note them on a sticky note for the days the memory resets. This two-minute task is the highest-leverage step in the entire purchase.

Standing posture still needs fitting

  • Elbows 90–110° at the standing preset — shoulders relaxed is the verification.
  • Monitor top edge at eye level in BOTH positions — dual-position monitor arms exist for a reason; a screen fitted for sitting is wrong for standing by ~20 cm for most people.
  • Anti-fatigue mat or soft shoes, weight shifts, one-foot-on-rail alternating.
  • Sit when tired. The ratio serves the body; forced records create the abandonment statistic.

The desk earns its price the day alternating becomes automatic: presets saved, ramp complete, movement anchored to calls and email. The trials' modest averages contain individuals with large wins — almost all of them the ones who set it up to be used.
